Forms Required

These forms will help you ensure that your research is done safely and in line with scientific ethics.

We require that you complete the forms that apply to your project, to document that you have complied with the rules that apply. Please read the following sections carefully, and complete all the forms you will need. Your project CANNOT be accepted at BASEF without them!

NOTE: Some forms must be completed and signed BEFORE you start experimenting, especially if you are using people or animals in your research!!!

1.BASEF Consent Form

(Required)
This consent form must be signed by one of the student's parents and a school sponsor. Each student exhibitor must submit this form.

2.BASEF Safety Form

This form is to be used as a guideline for you during the development of your project. On Setup Day March 25, 2009, a BASEF Safety Inspector will use this form to complete a safety check on the project you have setup.

3. Scientific Review Forms

(Required)

A. Junior Students

All Grade 7 & 8 students must print, complete and submit any applicable YSF Scientific review forms in order to enter BASEF and to be eligible for the Canada Wide Science Fair (CWSF).

  2. Designated Supervisor Form (Form YSF3)
  3. Form 4.1A: Participation of Humans - Low Risk - Approval Form and Instructions
  4. Form 4.1B: Participation of Humans - Significant Risk - Approval Form and Instructions
  5. Informed Consent - Permission Form
  6. Informed Consent - Letter of Information Sample
  7. Form 4.1C: Animals - Approval Form and Instructions
  8. Continuation Projects Form (Form YSF7)

B. Intermediate and Senior Students

All Grade 9 to 12 Students must print, complete and submit any applicable IISEF Scientific Review Forms.

NOTE:

Projects submitted by Grade 9 to 12 Students are eligible for both the Canada Wide Science Fair (CWSF) and also the Intel International Science & Engineering Fair (IISEF). To meet both CWSF and IISEF requirements, grade 9 to 12 students are required to comply with all the relevant rules, for BOTH YSF and ISEF.

MAIL FORMS

Mail all your signed Scientific Review Forms and BASEF Consent Forms. The envelope must be postmarked no later than Monday, March 1, 2010.
